What CYP Genetic Testing Can (and Can't) Tell Us
Patients often ask about genetic testing for medications. Usually they mean CYP testing, a cheek swab that checks how your liver enzymes process drugs. It sounds like a shortcut to the "right" medication. The reality is more limited.
As a child and adolescent psychiatrist practicing in Tribeca, NYC, and working with patients across the gender and sexual identity spectrum, including LGBTQIA+ youth and adults, this comes up often in consultations. Families and patients want clarity, and it's worth explaining what this testing can and can't actually offer.
What CYP enzymes actually do
CYP enzymes live in the liver. They break down most psychiatric medications, including SSRIs, SNRIs, and many antipsychotics. Two matter most in psychiatry: CYP2D6 and CYP2C19.
Your genes shape how active these enzymes are. Based on your variants, testing sorts you into a category:
Poor metabolizer. Breaks drugs down slowly. Levels can build up.
Intermediate metabolizer. Slower than average.
Normal metabolizer. Typical enzyme activity.
Rapid or ultrarapid metabolizer. Breaks drugs down fast. Levels may stay too low to work.
Where this is genuinely useful
Explaining an unusual reaction. If a patient gets severe side effects on a low dose, or feels nothing on a high one, a poor or ultrarapid metabolizer result can explain why. It turns a confusing case into a coherent one.
A few drugs with strong evidence. For some medications, like certain TCAs, or codeine's conversion to morphine via CYP2D6, the enzyme link to dosing is well established. CPIC (the Clinical Pharmacogenetics Implementation Consortium) publishes real, actionable guidelines for these.
Explaining, not predicting, past patterns. If someone has tried five SSRIs with poor tolerability, testing can sometimes make sense of the pattern retrospectively. This can matter in pediatric and adolescent cases, where medication trials are often more constrained and every data point counts.
Where it falls short
This is where the marketing gets ahead of the science.
Most psychiatric drugs don't have strong gene-outcome evidence. For the majority of antidepressants, CYP genotype explains only part of why a drug works or doesn't. Illness severity, adherence, dose, drug interactions, and plain biological variability outside the CYP system all matter more, in most cases, than genotype.
Commercial panels overstate certainty. Direct-to-consumer and combinatorial pharmacogenomic panels often package CYP results with serotonin transporter genes, folate metabolism genes, and proprietary scoring algorithms. The "green light / yellow light / red light" categories look authoritative. Much of that scoring isn't backed by strong trial data. The GUIDED trial and others have shown modest or inconsistent benefit from combinatorial testing in real-world outcomes.
A genotype doesn't equal a phenotype. Someone can carry "poor metabolizer" variants and still process a drug close to normal, because of enzyme induction, other medications, or pathways the test doesn't capture. Genetics is one input, not a verdict.
It doesn't replace clinical judgment. No panel tells you whether a patient's anxiety responds better to an SSRI or therapy first. It can't weigh in on dose titration speed, side effect tolerance, or whether a med trial failed because of the drug or because of an unaddressed stressor. That's the part of the work that still requires a clinician sitting with a full history, especially for children, adolescents, and LGBTQIA+ patients whose treatment context often involves more nuance than a lab result can capture.
The honest bottom line
CYP testing is a tool, not an answer. It's most useful after something unexpected has already happened: an unusual side effect, a nonresponse that doesn't fit the usual pattern. In that setting, it can help explain what's going on. It's much less useful as a first step for picking a medication, despite how it's often marketed.
If a patient is considering pharmacogenomic testing, the more useful question isn't "will this test tell me the right medication?" It's "what specific problem am I trying to solve, and does this test actually address it?" For most people starting a first medication trial, a careful history and a thoughtful conversation still outperform a cheek swab.
